IMPORTANT INFORMATION

Dear Patients:

By now you may have heard that Graduate Hospital will be closing and with it Graduate Pain Center.  Graduate Pain Center will continue its operations until the end of March and then close its doors.

We are writing this letter to inform you of our plans for the future.  Your doctors have partnered with Lankenau Hospital to create Lankenau Pain Center.  Lankenau hospital is located in Ardmore, PA.   Lankenau Hospital, part of the Jefferson Main line Health System, has an excellent reputation for clinical excellence and offers convenient location.  We feel fortunate that we have found a great hospital to continue to provide latest medical treatment as well as compassionate care for our patients.

Graduate Pain Center will continue our operation until March 30, 2007.  Lankenau Pain Center will commence its operation in May of 2007.   We have set up an interim office in Broomall for our patients until construction at Lankenau Pain Center is completed.  The interim office begins its operations on April 2, 2007.  Our Broomall office will be available for patients who can get to the office easily or requires urgent evaluation by one of our doctors.  The new phone number for both the interim office and the Lankenau Pain Center is 610-645-7000.  You can call this number to make an appointment or to contact clinical staff starting March 26, 2007.

We will make every effort to make the transition as smooth as possible for our patients.  We will also be updating our website with any new developments.  Please log on to www.graduatepaincenter.com for additional information and directions.

We apologize for the inconvenience but thank you for your understanding in this hectic yet exciting time.  If the new location is inconvenient for you and you wish to continue your care with another physician, please inform Graduate Pain Center of your decision so that arrangements can be made for the transfer of your medical records.

Should you have any questions or concerns, please contact us at 215-893-PAIN until March 30, 2007.  Afterwards, all questions should be directed to 610-645-7000.

 Sincerely,

Evan Frank, MD, PhD

John Park, MD
